Step 1: Click HERE to access our Online Enrollment Form.
On this form, you will be able to upload a picture of your government ID, your student’s birth certificate and your proof of residency documents.
If you don’t want to use DOCHUB, you may also download these documents below and mail, email or drop them off to the school:

Copy of Government-issued identification of person enrolling student

STEP 3: Email, Mail or Drop Off Records
DC Child Physical Health Certificate -Please see below about the new DC requirements for Immunizations
Copy of IEP/504 plan (If applicable)
Copy of student records/report card from previous schools – The report card review will determine the appropriate grade level and course placement. If a student does not meet the requirements for the grade level that they were accepted for, they may be placed back on our waiting list for the appropriate grade level.

DC Guidance- No Shots, No School
DC is now requiring that all students be up to date on their immunizations before the school year starts. HERE IS A LINK TO LOCATIONS WHERE STUDENTS CAN GET IMMUNIZATIONS.
(Dark Blue locations accept Medicaid and some private insurance plans. Light Blue locations only accept private insurance or self-pay.)
